| 摘 要： | 目的：总结胃十二指肠良性病变，非肝硬化引起消化道大出血手术治疗的经验。方法：对我院2009年1月-2010年10月间收治的16例良性病变致上消化道大出血手术患者的临床资料进行回顾分析。结果：本组16例病人术中行急诊胃镜检查，有10例出血部位为胃部，6例位于十二指肠球部，胃部出血均为血管破裂出血，十二指肠病变为十二指肠溃疡出血，其中3例合并幽门狭窄。10例胃部出血采取术中胃镜辅助下缝扎止血，6例球部溃疡出血采取术中胃镜辅助结合手触诊确定病变部位，局部全层缝合止血，不结扎胃十二指肠动脉：3例合并幽门梗阻病人行胃空肠吻合术，16例病人回访半年以上均未出现术后再出血。结论：术前采用胃肠减压及胃镜检查，有利于明确出血部位及原因；术中结合胃镜定位选择缝扎的手术方式对防止术后再出血非常重要。
