碱性成纤维细胞生长因子复合部分脱蛋白骨修复兔股骨头骨缺损的作用机理及相关性分析

摘    要:目的:探讨bFGF/PDPB在修复兔股骨头骨缺损过程中成骨与再血管化作用的相关性及其作用机理.方法:成年健康新西兰白兔24只48髋随机分为3组,每组16髋,建立股骨头骨缺损模型,A组植入bFGF/PDPB、B组植入PDPB、C组为空白对照.动物于术后2、4、8周分批处死,并制备墨汁灌注标本,进行组织学和微血管观察及血管密度图像分析,计数成骨细胞数和破骨细胞数,测定骨钙含量,并进行相关性分析.结果:A组骨缺损修复早于B和C组,C组术后8周缺损区仍存在;A组术后8周骨钙含量和血管密度高于B和C组;术后各时点血管密度与成骨细胞数成正相关;与破骨细胞于术后2周成正相关;与骨钙含量于术后2周呈负相关,术后8周呈正相关.结论:bFGF具有成骨和再血管化作用,促进移植材料的爬行替代,在治疗股骨头坏死方面具备一定的潜能和优势.

Abstract:Objective:To evaluate the effect of femoral head defect repairing with basic fibroblast growth factor b (bFGF) and partially deproteinised bone composite (PDPB),and to analyze the correlation between osteogenesis,angiogenesis and its mechanism.Method: After capsulotomy,a trapdoor was created in the area between the femoral head and the neck,and a 5mm diameter defect was established respectively in 48 femoral heads from 24 mature New Zealand rabbits,which were divided into three groups randomly and were implanted with bFGF/PDPB in group A,with PDPB in group B,and with no graft in group C respectively.The rabbits were killed after vessel injection with prepared Chinese ink in the 2th,4 th and 8 th week postoperatively,and then the femoral heads were taken off for observation.By this way,The bone repairing process was investigated.Histological and morphological observation of blood vessel changes,the counting of osteoblast and osteoclast cells ,the detection of calcium level ,and image analysis were performed subsequently.Result:The defects,in which bFGF/PDPB composite was implanted in group A,were repaired more rapidly than those with PDPB in group B,and the defects in group C still existed in the 8 th week postoperatively and were filled with fibrous tissue.Calcium content in the repaired bone tissue was higher in group A than those in other groups.The vascularization in group A was better than those in group B and C in the 2th and 4 th week.Positive correlation between the vessels density and the amount of osteoblast cells was significant in the 2th,4 th ,8 th week ,and positive correlation between vessels density and osteoclast was found at the 2th week as well.Negative correlation between the vessel density and calcium level was noticed in the 2nd week.Conclusion: Basic fibroblastic growth factor(bFGF) has the function of angiogenesis and osteogenesis,and can accelerate the creeping and substituting process of the implant in repairing of femoral head defect,thus has the potential in the treatment of avascular necrosis of femoral head.
