高血压脑出血开颅手术与微创手术的对比研究

摘    要:目的:对比高血压脑出血开颅手术与微创手术的临床疗效。方法:选取2006年3月~2009年5月收治入院的60例高血压脑出血患者,进行随机分组,微创组40例,开颅组20例,两组患者在意识状况上差异无统计学意义(P〉0.05)。微创组在局麻下行穿刺血肿碎吸术,开颅组行传统开颅血肿清除术。对比两组术后疗效。结果:两组术后疗效比较,微创穿刺血肿碎吸术治疗40例,基本治愈21例,显著进步8例,进步7例,无进步3例,死亡1例。开颅组20例,其中,开颅血肿清除16例,临床基本治愈7例,显著进步3例,进步2例,无进步3例,死亡1例;开颅去骨瓣4例,显著进步1例,进步1例,无进步1例,死亡例1例。结论:脑微创手术清除血肿,具有时间短、创伤小,再配合超早期手术能提高临床疗效。

Hypertensive cerebral hemorrhage and craniotomy comparison of minimally invasive surgery

Abstract:Objective:Craniotomy for hypertensive intracerebral hemorrhage compared with the clinical efficacy of minimally invasive surgery. Methods:March 2006 to May 2009 during the hospital treated 60 patients with hypertensive intracerebral hemorrhage patients were randomly divided 40 cases of invasive group,20 cases of craniotomy group,two groups of patients grouped on the situation awareness of the difference was not statistically significant (P0.05). Minimally invasive biopsy under local anesthesia group hematoma aspiration in surgery,craniotomy group was given traditional hematoma evacuation. Comparison of postoperative effect.Results:Comparison of postoperative efficacy of minimally invasive hematoma aspiration in treating 40 cases,21 cases were cured, 8 cases of significant progress,progress in 7 cases,no improvement was 3 cases,1 died.Craniotomy group,20 cases in which hematoma evacuation was 16 cases,including 7 cases of basic clinical cure;significant progress was 3 cases,2 cases of progress,no progress was 3 cases,1 died. 4 cases to the bone flap craniotomy;significant improvement was 1 case,1 case of progress,no progress was 1 case,1 case of death of patients.Conclusion:Minimally invasive surgical removal of intracerebral hematoma,a short time, trauma,coupled with the ultra-early surgery can improve the clinical results.
