含色氨酸的肠外营养对急性白血病患者免疫功能的影响

摘    要:目的观察含色氨酸的肠外营养对急性白血病患者免疫功能的影响。方法利用流式细胞术检测急性白血病患者外周血淋巴细胞亚群(CD3 、CD4 、CD8 、自然杀伤(NK)细胞及CD4 /CD8 )水平。结果急性白血病患者治疗前与健康对照相比较,CD3 T淋巴细胞、CD4 T淋巴细胞、NK细胞百分率以及CD4 /CD8 值均降低,差异有显著性(P<0.05);化疗后CD3 T淋巴细胞百分率较化疗前升高,但统计差异无显著性(P>0.05);CD4 T淋巴细胞、NK细胞百分率以及CD4 /CD8 比例也升高,与化疗前相比较,差异有显著性(P<0.05);在化疗的同时予以含色氨酸的肠外营养治疗后,CD3 T淋巴细胞、CD4 T淋巴细胞、NK细胞百分率以及CD4 /CD8 比例与联合治疗前相比较均升高,差异有显著性(P<0.05);单纯化疗组和联合治疗组比较,CD3 、CD8 T淋巴细胞以及NK细胞百分率两组之间无显著性差异(P>0.05),CD4 T淋巴细胞百分率和CD4 /CD8 比例联合治疗组高于单纯化疗组,差异有显著性(P<0.05)。结论急性白血病患者外周血淋巴细胞亚群水平存在异常,通过化疗可以得到改善。在化疗的同时予以含色氨酸的肠外营养可以更进一步改善患者的免疫功能。

Abstract:Objective To study the effects of parenteral nutrition containing tryptophan accompanied with chemotherapy on immunity of patients with acute leukemia. Methods The ratios of lymphocyte subpopulations (CD3 ,CD4 ,CD8 ,natural killer(NK)cells and CD4 /CD8 ) in peripheral blood were analyzed by flowcytometry. Results Compared with the ratios of lymphocyte subpopulations in healthy controls, CD3 ,CD4 ,NK subsets and CD4 /CD8 in acute leukemia patients significantly decreased .After chemotherapy ,CD4 ,NK subsets and CD4 /CD8 of patients increased,but CD3 and CD8 subsets showed no difference.When parenteral nutrition containing tryptophan were injected during chemotherapy, the ratios of CD3 ,CD4 ,NK subsets and CD4 /CD8 obviously increased compared with that of patients before treatments,and the ratios of CD4 and CD4 /CD8 increased compared with that of patients only treated by chemotherapy. Conclusions The patients with acute leukemia have immunoimpotent,which could be improved through chemotherapy, while the parenteral nutrition containing tryptophan could further improve the immunity of patients with acute leukemia.
