动脉瘤样骨囊肿的CT诊断及鉴别诊断

目的探讨CT对动脉瘤样骨囊肿(aneurysmal bone cyst,ABC)的诊断价值及其鉴别诊断。方法回顾性分析40例经手术病理证实的ABC的CT表现。结果40例ABC发生于长骨28例,脊柱9例,骨盆3例。表现为溶骨性骨质破坏,常有不同程度膨胀性改变,骨质破坏区内可见囊状低密度及骨性分隔。膨胀的骨壳内缘呈清晰弧形压迹,其中骨壳完整30例;5例见病灶周软组织肿块;35例见蜂房样低密度影;3例出现液—液平面。结论CT对ABC的诊断及鉴别诊断有重要的价值。

CT Diagnosis and Differential Diagnosis of Aneurysmal Bone Cyst

Objective To investigate the CT diagnosis value and differential diagnosis for aneurysmal bone cyst （ABC）. Methods CT manifestations of ABC patients who were confirmed by pathology were analysis retrospectively. Results In 40 ABC cases, 28 cases genesis from longum, 9 cases from spine, 3 cases from pelvis. The feature showed osteolytic destruction, often expansive destruction, It is showed cystic low density and bone septations in bone destruction region. The lesion was characterized by the obvious arced impression on the inner side of bony shell, while the bone shell was integrated （ n = 30） or broken. 5 cases can find soft tissue tumor around the lesions, 35 cases had low density sign as ＂honeycomb＂ and 3cases had fluid - fluid level. Conclusion CT had important diagnosis and differential diagnosis value in ABC.
