颈动脉内膜剥脱术后即期脑中风的病因、影响因素及治疗

对２９１例颈动脉内膜剥脱术后患者进行随访研究，１例术后即期死亡；２２例（６．３％）在术后发生脑中风，１７例为中度中风，５例为严重中风，即期中风的病因包括：１４例手术部位颈动脉血栓形成（１４／２２，６４％），４例术中或术后即期脑栓塞，２例阻断颈动脉所致脑缺血，１例脑出血，１例原因不明。此外讨论了术后中风的危险因素和处理方法。

Etiology , Risk Factors and Management of Perioperative Stroke of Carotid Endarterectomy

The first author had visited and worked forone year at Monash Medical Centre in Australia ,who re-viewed and researched all the history of29 1 patients whounderwent carotid endarterectomy from January1,1987to June30, 1992at Monash Medical Centre and form January1,1985 to June30,1991 at Prince Herry' s Hcopitalin Australia,The perioperative stroke and mortal ity ofcarotid enderwest carotid erdarterectomy from January1,1987 to June 30,19928 at Monash Medical Centre andfrom January1,1985 to June30, 1991 at Prince Herry'SHospital in Australia. The etiology of t he perioporativestroke were due to thrombosis of the operative site in 14eases(14/ 22 ,64%) ,due to carebral embolization duringor immediately postoperatively in 4 cases .due to clampingischemia in 2 cases, due to intracerebral hemorrhage in 1case,and the cause was not clear in one case,The risk fac-tors and management of perioperative stroke had been d is-cussed in this paper。
