咽后壁瓣术后形态变化的临床研究

目的　研究上、下蒂型咽后壁瓣对腭咽闭合功能及患者发音的影响。方法　采用鼻咽纤维镜、头影测量及语音评价的方法 ,对咽后壁瓣的形态、腭咽闭合功能及语音清晰度进行对照定量分析。结果　上蒂型咽后壁瓣腭咽闭合不全率及矢状收缩不全率均小于下蒂型咽后壁瓣 ,两者的语音清晰度无明显区别。结论　上蒂型咽后壁瓣术式优于下蒂型咽后壁瓣术式 ,且患者术后均应进行系统的语音评估及治疗

THE CLINICAL EVALUATIVE INVESTIGATION OF THE POSTOPERATIVE MORPHOLOGICAL CHANGES OF POSTERIOR PHARYNGEAL FLAP

Objective This experiment attempts to investigate the effects of the superiorly and inferiorly based posterior pharyngeal flap on velopharyngeal closure and speech. Methods The examinations of nasopharyngeal fiberscope, cephalometer and speech assessment were performed to compare the differences among the postoperative three dimensional morphological changes?volume changes and clinical outcomes. Results The RVPI (Rate Velopharyngeal Insufficent) and RVSR (Rate Velopharyngeal Coronary Restriction) of superiorly based pharyngeal flap were lower than that of inferiorly. Indices of Chinese articulation clear degree showed no significance between the superiorly and the inferiorly based posterior pharyngeal flap. Conclusion The superiorly based pharyngeal flap was a better method than the inferiorly based pharyngeal flap. After the operation, patients with or without VPI need speech evaluation and speech therapy.
