58例急性胆源性胰腺炎外科治疗分析

目的：探讨急性胆源性胰腺炎（AGP）的治疗方法。方法：回顾性分析58例AGP患者的临床资料,轻症急性胆源性胰腺炎（MAGP）46例,急诊后期手术2例,非手术治疗44例;重症急性胆源性胰腺炎（SAGP）12例,均行急诊手术。结果：本组病例均痊愈,无死亡。结论：按照AGP病因、临床表现和分型选择恰当的治疗方法。对伴有化脓性胆管炎和梗阻型SAGP者早期宜行急诊手术,其它类型AGP早期宜非手术治疗,后期治疗胆石病,可以降低病死率,减少并发症的发生。

Analysis on surgical treatment of acute gallstone pancrentitis of 58 cases

Objecteve: To explore the characteristics of the diagnosis and treatment in patients with acute gallstone pancreatitis (AGP) and improve the level of the diagnosis and treatment of AGP. Methods:We retrospectively analysed the data of 58 cases of AGP in our hosptital. 46 cases of mild acute gallstone pancreatitis, emergency surgery was performed in two cases, 44 cases with non-surgical therapy. In 12 cases of severe acute gallstone pancreatitis and all received emergency surgery. Results: All patients were cured with no death. Conclusions: According to its etiology, clinical features and classification of acute gallstone pancreatitis the appropriate method of treatment should be chosen. Associated with suppurative cholangitis and obstruction of severe acute gallstone pancreatitis who are advised early emergent surgery early gallstone pancreatitis are advised to other types of non-surgical treatment.
