全麻并硬膜外阻滞对开胸术老年患者肺功能的影响

【目的】探讨全麻气管插管术合并硬膜外阻滞对开胸手术的老年患者早期肺功能的影响。【方法】将46例老年胸科手术患者，随机分为试验组（A组，n=23）和对照组（B组，n=23）A组用全麻加硬膜外阻滞，B组仅用全麻。观察两组患者的肺活量△VC（L）、二氧化碳分压△PaCO2（mmHg）、动脉血管分压△PaO2（mmHg）。【结果】两组病例术后镇痛评分、肺功能指标和对照组比较差异有统计学意义。【结论】全麻合并硬膜外阻滞术后有利于早期咳嗽排痰，促进肺功能恢复。

Effect of General Anesthesia and Epidural Anesthesia on Pulmonary Function in Elderly Patients Undergoing Thoracotomy

[Objective]To explore the effect of general anesthesia combined with epidural block and endotracheal intubation on early lung function in elderly patients undergoing thoracotomy. [Methods]Forty six elderly patients undergoing thoracic surgery were randomly divided into test group （group A, n=23） and control group （group B, n=23）. Group A received general anesthesia plus epidural block and group B only received general anesthesia. Lung capacity, partial pressure of carbon dioxide and arterial partial pressure in two groups were measured. [Results]There was significant difference in postoperative pain score and pulmonary function between two groups. [Conclusion] General anesthesia combined with epidural block is better for early postop erative expectoration and improvement of pulmonary function.
