免疫印迹法检测血清幽门螺杆菌抗体的应用价值

目的评价免疫印迹法检测幽门螺杆菌（H.pylori）抗体的临床性能,探讨免疫印迹法检测血清幽门螺杆菌抗体和免疫学分型的临床意义。方法应用免疫印迹法检测360例上消化道疾病患者和20名无消化道疾病的健康志愿者血清幽门螺杆菌空泡毒素（VacA）、细胞毒素（CagA）和尿素酶（Urea）等抗体。根据幽门螺杆菌抗体的表达进行幽门螺杆菌Ⅰ型（毒力型）和Ⅱ型（非毒力型）免疫学分型,并结合14C-UBT尿素呼气试验进行分析。结果免疫印迹法诊断幽门螺杆菌感染的敏感性为96.7%,特异性为40.4%。免疫印迹法检出该组幽门螺杆菌抗体阳性率为81.7%。其中幽门螺杆菌Ⅰ型阳性率（53.7%）高于对照组（25.0%）,而Ⅱ型阳性率（28.0%）低于对照组（50.0%）。结论免疫印迹法检测血清幽门螺杆菌抗体在幽门螺杆菌的流行病学调查、血清学诊断和免疫学分型等方面具有独特的应用价值。

Detection of Helicobacter pylori serum antibodies by immunoblotting

Objective To assess the clinical performance of Helicobacter pylori （HP） antibodies detection by immunoblotting （IB） assay,and investigate the clinical significance of Helicobacter pylori serum antibodies detection by IB.Method The VacA,CagA,Ureas serum antibodies for HP of 360 patients who had disease of upper digestive tract and 20 healthy volunteers without symptoms in the control group were detected by immunoblotting assay,and also combined with 14 C Uneabreath test.Results The sensitivity and specificity of HP infection by immunoblotting assay were 96.7% and 40.4%,respectively.The positive rate of type I HP （virulence type） was 53.7%,which was higher than control group （25.0%）,while type II HP （non virulence type） was 28.0%,which was lower than the control group （50.0%）.The total positive rate was 81.7%.Conclusion Detection of HP serum antibody by IB had an unique value on HP epidemiological investigation,serodiagnosis and immunological type.
