微导管介入治疗小肝癌的临床研究

目的:评价微导管介入治疗小肝癌的临床效果.方法:86例小肝癌患者,经影像学、病理学或细胞学证实,单个病例病灶数目≤2个,2个病灶直径之和≤3 cm;共发现病灶112个；使用微导管超选择技术共进行217次节段性化疗栓塞术.其中多发者和多支动脉供血者,行多支供血动脉超选择性栓塞术.结果:病灶直径≤3 cm者42例,3 cm＜直径≤5 cm者44例；86例原发性小肝癌病灶内碘油沉积完全、密实,有11例手术切除,术后病理显示病灶中心组织全部坏死.术后1、3、5a生存率分别为94％、76％、48％.肝功能损害轻微.结论:微导管介入治疗小肝癌疗效显著,是治疗小肝癌安全、有效的主要手段之一.

Clinical Research of Microcatheter Interventional Therapy to Small Hepatocellular Carcinoma

Objective To discuss the clinical effects of microcatheter interventional therapy for small hepatocellular carcinoma.Methods Eighty-six cases of small liver cancer patients were verified by imaging,pathology or cytology,with the number of focuses not more than 2 and the summation of the two diameters not more than 3 cm.There were 112 focuses found,and 217 times of periodical chemoembolization were performed.The patients with multiple carcinomas and multi arterial lines underwent multi-artery selective embolization.Results There were 42 ones with the focus diameter not more than 3 cm,44 ones between 3 cm and 5 cm,and 86 ones of primary small liver cancer with iodipin depositing completely.Eleven ones went through exairesis,and total necrosis happened in the central tissue of the focus.The survival rate one day,3 days or 5 days after the operation were 94%,76% and 48% respectively,with light liver function lesion.Conclusion Microcatheter interventional therapy is effective for small hepatocellular carcinoma.
