食管鳞癌患者组织和血清中VEGF-C的表达

目的:检测食管鳞癌患者组织和血清中VEGF-C的表达,探讨其在判断淋巴结转移中的作用.方法:62例食管鳞癌患者和10例正常人的血清,用酶联免疫吸附法检测血清中VEGF-C的含量,用免疫组织化学法检测食管癌组织中VEGF-C的表达,并进行ROC曲线分析.结果:食管鳞癌组织中VEGF-C在有淋巴结转移者的阳性表达率高于无转移者(P=0.015),肿瘤浸润深度T3者阳性表达率高于T1-2者(P=0.039);组织学检查诊断淋巴结转移的灵敏度为55.5%,特异性为76.1%,准确率为67.8%.食管鳞癌患者血清VEGF-C的水平高于正常人(P＜0.0001),有淋巴结转移者高于无转移者(P＜0.0001),肿瘤浸润深度T3者高于T1-2者(P=0.009);如果血清VEGF-C水平以2 474.5 ng/L为界值,则诊断淋巴结转移的灵敏度和特异性分别为85.0%和73.8%,准确率77.4%.结论:血清VEGF-C检测是判断食管鳞癌淋巴结转移的可靠标志物.

Expression of VEGF-C in tissue and serum of esophageal squamous cell carcinomas patients

Aim: To detect the expression of VEGF-C in tissue and serum of human esophageal squamous cell carcinoma (ESCC) and assess its role as a diagnostic tool for determining lymph node metastasis. Methods: Sixty-two patients with ESCC and ten healthy control were included in the current study. VEGF-C levels in serum were assessed by enzyme-linked immunosorbent assay.VEGF-C in ESCC tissue was detected using immunohistochemistry.Clinical data were obtained from medical records for ROC analysis. Results: Serum VEGF-C concentration was higher in patients with ESCC than that in healthy control (P<0.000 1). Patients with lymph node metastasis revealed higher serum VEGF-C than those without(P<0.015). According to the depth of primary tumor, the level of VEGF-C was higher in the group with T3 than that in the group with T1-2(P=0.039). In diagnosis of the lymph node metastasis according to VEGF-C levels in tissue,the sensitivity was 55.5%,specificity was 76.1%,and accuracy was 67.8%.Serum VEGF-C levels reached the highest sensitivity and specificity in diagnosing lymph node metastasis when a cutoff value of 2 474.5 ng/L was applied (sensitivity was 85.0%, specificity was 73.8%, and accuracy was 77.4%).Conclusion: Serum VEGF-C is a reliable marker for diagnosing lymph node metastasis in ESCC.
