中心静脉导管置入胸腔治疗结核性胸膜炎临床分析

目的探讨中心静脉导管置入胸腔持续引流替代反复多次胸腔穿刺抽液治疗结核性胸膜炎临床应用价值。方法对我院2008年1月至2012年12月收治的经胸部X线或胸部超声检查确诊为中等或大量胸腔积液的结核性胸膜炎病例80例,随机分为两组（治疗组40例,对照组40例）,分别用中心静脉置管持续引流和反复多次的胸腔穿刺抽液治疗,对这两种方法的效果、并发症情况及住院费用进行比较。结果两种方法相比：胸水吸收时间、胸膜反应、胸水包裹发生率、气胸发生率,住院费用、治疗后3个月的胸膜厚度等各项指标均有显著性差异（P〈0.05）。结论中心静脉导管置入胸腔持续引流治疗结核性胸膜炎方法简单、安全、疗效好、经济、患者痛苦少,值得推广。

Exploration on clinical application of central venous catheter pleural for tuberculous pleurisy

Objective To explore the clinical application of central venous catheter pleural instead of repeated pleural puncture and drainage for tuberculous pleurisy. Methods 80 tuberculous pleurisy patients were diagnosed by the chest X-rays or ultrasonic inspection from January 2008 to December 2011. They were randomly divided into 2 groups（ 40 cases in treatment group,40 cases control）. They were treated by central venous catheter for continuous drainage or fluid drained more than once. Compared to the effects,complications and hospitalization costs. Results There were significant differences with the absorption time of pleural effusion,reaction of pleural,and the incidence of pleural fluid inclusions,hospitalization costs and the thickness of pleura after 3 months of treatment. Conclusions The method is simple,safe,effective,economic,less pain of patients,and is worthy of promotion.
