25例下颌发育不全的外科-正畸联合矫治

目的：总结应用外科-正畸联合矫治下颌发育不全畸形的效果。方法：2004～2008年收治的25例下颌后缩和小颌畸形。按照术前正畸→手术→术后正畸的序列,分别对20例下颌后缩进行术前术后正畸联合各种正颌手术,对5例重度小颌畸形行牵张（引）成骨术（DO）。结果：通过术后正畸和随访,本组病例中20例（80%）颌面外貌明显改善,咬合关系良好;其余5例（20%）颌面外貌和咬合改善。结论：坚持外科-正畸联合是取得佳效果的关键;轻度下颌后缩可行正畸掩饰治疗;SSRO是前徙下颌的首选术式;DO对矫治重度小颌畸形为优选。

Combined Surgery with Orthodontic Therapy for Mandibular Hypoplasia: Analysis of 25 Cases

Objective： To summary the effects of 25 cases who suffer from mandibular hypoplasia accepting surgery and orthodontic treatment.Methods： A total of 25 patients were treated in our hospital between 2004 and 2008.20 patients suffered from mandibular retrognathism,5 patients suffered from servers micrognathia accompanying OSAHS.According to the standard procedure,20 patients were treated by presurgical orthodontic treatment,surgery and postsurgical orthodontics.5 patients were treated by distraction osteogenesis.Results： Through evaluating clinical and cephalometric data,the occlusal relationship and profile were significantly improved in 20（80%）patients and some improved in 5 patients（20%） postsurgery.Conclusion： Combination of surgery and orthodontic treatment is the key of achieving good effects.Orthodontic treatment can be used on slight mandibular retrognathism.SSRO is the best choice for mandible advancement;DO is the best choice for moderate and severe micrognathia.
