降钙素原在成人心脏术后感染的监测价值

目的：探讨成人心脏术后早期发生呼吸道感染患者围术期降钙素原的变化。方法连续收录该院2016年1-5月行心脏手术，年龄大于18岁的患者286例，其中35例在心脏术后72 h内发生呼吸道细菌感染的患者(感染组)和251例未发生感染的患者(非感染组)于术前、手术结束、术后24、48、72 h分别检测血浆降钙素原水平。结果非感染组患者手术结束时(1.18±0.07)ng/mL、24 h(2.36±1.45)ng/mL、48h(1.78±0.98)ng/mL、72 h(1.47±0.65)ng/mL及感染组手术结束时(1.65±0.44)ng/mL、24 h(5.59±1.77)ng/mL、48h(6.74±3.54)ng/mL、72 h(8.43±3.36) ng/mL水平明显高于术前(0.05±0.01)ng/mL(P<0.05)；感染组术后24、48、72 h血浆降钙素原水平明显高于非感染组(P<0.05)。结论心脏术后早期发生呼吸道细菌感染者的血浆降钙素原水平增高。

Value of Procalcitonin to Monitoring Infection in Adult Cardiac Postopera-tive

Objective To investigate the change of procalcitonin in adult with respiratory tract infection in the early period after the cardiac postoperative.Methods The procalcitonin level was detected before the cardiac operative, at the end of surgery, postoperative 24 h, 48 h, 72 h in 286 patients with over 18 years of age,including 35 cases in cardiac surgery pa-tients with respiratory tract bacterial infections occurred within 72 hours(infection group) and 251 cases of patients who were not affected (control group) during in the first five months 2016. Results The procalcitonin level was significantly higher at the end of surgery(1.18±0.07) ng/mL, postoperative 24 h(2.36±1.45) ng/mL、48 h(1.78±0.98) ng/mL、72 h(1.47±0.65) ng/mLin uninfection group and the end of surgery(1.65±0.44) ng/mL, postoperative 24 h(5.59±1.77) ng/mL、48 h(6.74±3.54) ng/mL、72 h (8.43±3.36) ng/mL in infection group than the preoperative (0.05±0.01) ng/mL (P<0.05);The procalcitonin level was significantly higher in infection group in 24, 48, 72 h after operation than uninfected group (P<0.05). Conclusion The pro-calcitonin level increases in the early period after the cardiac postoperative with respiratory tract bacterial infections.
