纤维支气管镜辅助治疗重症呼吸机相关性肺炎的疗效及对呼吸力学指标的影响分析

摘    要:目的:对纤维支气管镜辅助治疗重症呼吸机相关性肺炎(VAP)的疗效及对呼吸力学指标的影响予以探究并剖析。方法随机选取该院2013年2月—2015年4月期间收治的重症呼吸机相关肺炎患者进行研究,随机分为观察组与对照组,各64例,对照组组采用常规灌洗,观察组采用纤维支气管镜肺泡灌洗,对两组患者治疗情况及呼吸力学参数治疗进行探讨。结果从治疗有效率方面分析,观察组和对照组分别为:96.87%,79.69%,组间差异用χ2检验后,证明(P﹤0.05);从呼吸力学参数指标方面分析,观察组治疗后的改善情况显著优于对照组,组间差异用t检验后,证明差异有统计学意义(P﹤0.05)。结论为了提高临床治疗效率,改善呼吸力学指标参数,促进肺的复张和控制肺部感染,建议临床为重症VAP患者实施常规抗感染联合纤维支气管镜肺泡灌洗治疗方案。

Fiberoptic Bronchoscopy Auxiliary Treatment of Severe Ventilator Associated Pneumonia Curative Effect and Impact on Respiratory Mechanics Index anal-ysis
Abstract:Objective To fiber bronchoscope auxiliary treatment of severe the curative effect of ventilator associated pneu-monia (VAP) and to explore and analyze the impact on the respiratory mechanics indexes. Methods Randomly selected from our hospital in February 2013 to April 2015 entilator-associated pneumonia admitted during the period of severe patients, randomly divided into observation group and control group, all the 64 cases, control way with normal irrigation, observation group with fiber bronchoscope, alveolar lavage treatment and respiratory mechanical parameters in patients with two groups of treatment were discussed. Results Analysis from the aspects of treatment outcome, the observation group and control group, respectively, 96.87%, 79.69%, the difference between groups with X2 test, prove the difference is statistically sig-nifceant(P﹤ 0.05); From the aspects of respiratory mechanics parameters analysis, the improvement of observation group af-ter treatment was superior to that of control group, the difference between group by t test, prove the difference is statistically signifceant(P﹤0.05). Conclusion In order to improve the efficiency of clinical treatment, improve the respiratory mechanics parameters, promote lung after zhang and control of pulmonary infection, suggested that clinical for critically ill patients with VAP routine anti-infection combined fiber bronchoscope alveolar lavage treatment plan.
