腹腔镜根治术治疗结肠癌的应用效果及预后初步研究

目的：研究腹腔镜根治术治疗结肠癌的应用效果。方法研究资料随机选取于该院2014年4月一2015年10月期间收治结肠癌患者100例,随机将患者分为2组,腹腔镜组和开腹组,每组有50例。开腹组采取的为开放手术；腹腔镜组行腹腔镜根治术治疗。就两组患者操作时间、术中出血、淋巴结清扫个数、术后恢复排气、恢复正常体温、下床活动时间和手术并发症发生率进行比较。结果腹腔镜组手术并发症发生率6.00%低于开腹组18.00%,P＜0.05。腹腔镜组患者操作时间、淋巴结清扫个数(172.74±11.85）min、(10.02±0.53）个跟开腹组(177.14±10.91）min、(10.98±0.72）个差异无统计学意义,P＞0.05。腹腔镜组患者术中出血、术后恢复排气、恢复正常体温、下床活动时间(153.52±24.92）mL、(9.02±0.53）h、(32.46±6.35）h、(1.52±0.72）h均少于开腹组(225.26±44.57）mL、(41.11±0.62）h、(74.56±11.89）h、3.65±0.57）h,P＜0.05。结论腹腔镜根治术治疗结肠癌的应用效果及预后良好,可跟开腹手术一样有效清除淋巴结,手术创伤小,可加速术后康复,减少并发症的发生,对患者预后有益,值得推广。

A Preliminary Study on the Effect and Prognosis of Laparoscopic Radical Operation in the Treatment of Colon Cancer

Objective To study the effect of laparoscopic radical operation in the treatment of colon cancer. Methods Ran-dom selection 100 cases of colon cancer patients were randomly divided into 2 groups, laparoscopic group and laparotomy group, 50 cases in each group. The data were from October 2015 to April 2014 in our hospital. Open surgery was performed in the open group and the laparoscopic group was treated with laparoscopic radical operation. The operation time, bleeding, the number of lymph node dissection, postoperative recovery exhaust, the normal body temperature, the time of operation and the incidence of postoperative complications were compared between the two groups. Results The incidence of compli-cations in the laparoscopic group was 6% lower than that in the open group 18%, P< 0.05. Laparoscopic group operation time, the number of lymph node dissection(172.74±11.85)min,(10.02±0.53) with open group (177.14±10.91) min, (10.98± 0.72) no significant difference, P> 0.05. In the laparoscopic group of patients with hemorrhage, postoperative recovery ex-haust, body temperature returned to normal, ambulation time (153.52 ±24.92)mL, (9.02 ±0.53)h, (3.246 ±6.35)h, (1.52 ± 0.72)h were less than laparotomy group (225.26±44.57)mL, (41.11±0.62)h, (74.56±11.89)h, (3.65±0.57)h, P< 0.05. Conclusion: laparoscopic radical surgery in the treatment of colon cancer application effect and the prognosis is good, with open surgery as effective removal of lymph nodes, small operation wound, can accelerate the operation after rehabilitation, reduce complications, beneficial to the prognosis of the patients, worthy of promotion.
