前后联合入路双钢板固定治疗复杂胫骨平台骨折的疗效分析

目的：探讨前后联合入路双钢板固定治疗复杂胫骨平台骨折的疗效。方法方便选取该院2013年8月—2015年11月期间收治的106例复杂胫骨平台骨折患者，按照双盲法将其分为两组。对照组患者仅采用单纯前正中入路固定治疗，观察组患者采用前后联合入路双钢板固定治疗。观察两组患者手术情况，采用Rasmussen膝关节功能评分对两组患者治疗前后关节功能改善情况进行评分。结果观察组患者手术时间(88.2±18)min、术后引流量(117.8±32)mL、术后并发症发生率5.67%均显著优于对照组(96.4±20)min、(134±34)mL、15.09%，差异有统计学意义(P﹤0.05)。观察组患者治疗后膝关节功能评分(23±8.2)显著优于治疗前(12±4.5)，对照组治疗后膝关节功能评分(18±7.6)显著优于治疗前(13±5.2)，观察组治疗后显著优于对照组，差异有统计学意义(P﹤0.05)。结论前后联合入路双钢板固定治疗复杂胫骨平台骨折的效果较好，不仅能快速改善患者关节功能，且并发症较少，值得推广使用。

Anaysis of Clinical Effect of Anterior and Posterior Approaches Combined with Double-plating Internal Fixation in Treatment of Complex Tibial Plateau Fracture

Objective To probe into the clinical effects of anterior and posterior approaches combined with double-plating internal fixation in treatment of complex tibial plateau fracture. Methods Convenient selection 106 patients with complex tibial plateau fracture and who accepted treatments in our hospital from August 2013 to November 2015 were selected, and according to the double blind method were divided into two groups.In the control group of patients were only treated with anterior median approach while in the observation group of patients were treated with anterior and posterior approaches combined with double-plating internal fixation.Then,the operation condition of two groups of patients were observed,and the improvement of joint function before and after treatment of two groups of patients were scored by the Rasmussen knee func-tion score.Results The operation time of (88.2±18)min, postoperative drainage volume of(117.8±32)mL, postoperative com-plications of 5.67% of patients in the observation group were significantly better than those in the control group of (96.4± 20)min、(134±34)mL、and 15.09%, the difference was statistically significant (P<0.05).The knee function score of (23±8.2) in the observation group was significantly better than that before treatment of(12±4.5),and the knee function score of(18±7.6) in the control group was significantly better than that before treatment of (13±5.2),After the treatment, the observation group was significantly better than the control group, the difference was statistically significant (P<0.05).Conclusion The clini-cal effects of anterior and posterior approaches combined with double-plating internal fixation in treatment of complex tibial plateau fracture are good, which not only can improve the patient's joint function, andthe complications are less, thus it is worthy to be promoted.
