A型链球菌制剂联合顺铂治疗恶性胸水疗效分析

目的探讨胸腔内注入A型链球菌制剂和顺铂治疗恶性胸水的方法和疗效。方法经病理细胞学证实的恶性胸腔积液49例,经采用中心静脉管行胸腔引流术排尽胸腔积液后,随机分两组,治疗组：26例患者采用胸腔内注入顺铂及A型链球菌制剂,对照组：23例患者仅用顺铂胸腔内注入,2周后复查,最多3次观察疗效及毒副反应。结果总有效率：治疗组为84.6%,明显高于对照组的52.2%（P〈0.05）,治疗组仅发热较对照组多,但差异无统计学意义（P〉0.05）。而胃肠道反应,骨髓抑制差异有统计学意义（P〈0.05）。结论A型链球菌制剂与顺铂联合腔内注射治疗恶性胸水是有效低毒可行的方法,较单药顺铂疗效好。

Effect of Combination of OK-432 and Cisplatin on Malignant Pleural Effusion

Objective To explore curative effect of combination of OK-432 and cisplatin in treatment of malignant pleural effusion. Methods 49 patients with malignant pleural effusion were divided into therapy group and control group randomly. 26 patients were treated with OK-432 and cisplatin by intrapleural injection in treatment group. 23 patients were treated with cisplatin only by intrapleural injection in control group. The changes in volume of pleural effusion and treatment effects were observed in two groups. Results The effective rate in treatment group and control group were 84.6% and 52.2% respectively（P 〈 0.05 ）. The side effects of treatment group were fever, chest ache, arrest of bone marrow and digestive tract reaction, but there was no obvious difference between the two groups （P 〉 0.05）. Conclusion The intrapleural injection with OK-432 and cisplatin is simple and effective in treatment of malignant pleural effusion, and it is better than that only cisplatin.
