社区高血压患者综合干预效果评价

目的：评价社区高血压患者综合干预效果。方法：对自愿加入综合干预组的无锡市华庄社区82例高血压患者，进行为期一年的综合干预，开展问卷调查、持续强化高血压相关知识健康教育、体格检查、个体化用药指导、定期血压监测以及血生化检查等指标，比较干预前后患者高血压知晓率、治疗率、控制率。结果：干预后高血压患者收缩压和舒张压分别为（135.93±11.63）mm Hg和（86.12±9.36）mm Hg，均低于干预前的（145.01±12.58）mm Hg和（93.16±8.38）mm Hg（P〈0.001）；高血压知晓率、治疗率和控制率分别为81.71%、90.24%和84.15%，较干预前的62.20%、71.95%和63.41%提高（P〈0.05）。结论：综合干预措施可以使患者血压水平控制良好，提高患者高血压知晓率、治疗率和控制率，促进患者建立健康生活方式。坚持社区高血压综合干预可使高血压患者获益。

Evaluation of A Community-based Integrated Intervention on Patients with Hypertension

Objective：To evaluate the effects of community-based integrated intervention on patients with hypertension in Huazhuang district in Wuxi.Method：An integrated intervention was administered to voluntary participants （n=82）over the course of one year.Participants were given a matter investigate,sustained intensify healthy education about hypentension knowledge,guided drug using individual,a physical examination and a laboratory blood test to compare the following before and after the intervention：level of awareness,treatment adherence,treatment effectiveness, life habits,blood pressure,fasting blood glucose and blood lipid. Result：After the intervention the average systolic blood pressure（SBP）and average diatolic blood pressure（DBP）of participants were significantly decreased from （145.01±12.58）mm Hg and （93.16±8.38）mm Hg to（135.93±11.63）mm Hg and（88.12±9.36）mm Hg, respectively（P〈0.001）.The levels of awareness,treatment adherence and treatment effectiveness were significantly increased from 62.20%,71.95%and 63.41%to 81.71%,90.24%and 84.15%,respectively（P〈0.05）. Conclusion：Developing an integrated intervention including follow-up classification management,encouragement of self-management and family management,and promotion of education about hypertension can effectively decrease blood pressure among persons with hypertension,improve the level of awareness,improve treatment and control of hypertension,and promote a healthy life style.To insist on the intervention for patients with hypertension can give more medical healthy-care.
