糖尿病合并胆石症围术期持续皮下注射胰岛素临床研究

摘    要:目的研究糖尿病合并胆石症患者围术期持续皮下注射胰岛素的可行性及血糖稳定对患者围术期危险性的影响。方法在我院40例2型糖尿病合并胆石症需手术治疗的患者中随机选取20例使用胰岛素泵(CSII)和20例常规皮下胰岛素注射治疗(MSII组)患者,比较两组患者治疗前后血糖、血糖达标时间、胰岛素用量及术后并发症的发生率。结果两种治疗方法皆能有效控制血糖,CSII组血糖达标时间明显短于MSII组,胰岛素用量及术后并发症的发生率CSII组明显少于MSII组。结论使用胰岛素泵持续皮下输注胰岛素的方式模拟生理胰岛素分泌,从而将围术期血糖在短期内控制到满意水平,可以缩短住院日,减少手术并发症。

Abstract:Objective To study the feasibility of coutinuous insulin subcutaneous infusion and the effect of blood sugar stability to the prognosis(fatalness) of diabetic patients combined with cholecystitis in the perioperative period.Methods 40 patients of T2MD combined with cholecystitis were randomly divided into groups of continuous subcutaneous insulin infusion(CSII,n=20) and multiple subcutaneous insulin injection(MSII,n=20).The blood glucose,the dose of insulin and the incidence rate in two groups were compared.Results Both methods were effective in controlling blood glucose,while the time consuming by standardizing the control of blood glucose in CSIIgroup was shorter than that of MSII group obviously,and the insulin dose needed in CSII group was less than that in MSII group and the incidence rate of operation complication in the former group was less than that in the later.Conclusion The method of continuous insulin subcutaneous infusion can imitate the "physiological insulin secretion",so that the blood sugar in the perioprative period can be adjusted to the satisfying level in a short time,and the time of hospitalization and the operative complication could be reduced.
