血清胃蛋白酶原含量变化对幽门螺杆菌相关性胃炎治疗效果评价

目的 探讨血清胃蛋白酶原(pepsinogen,PG)I、Ⅱ含量变化对幽门螺杆菌(Helicobacter pylori,Hp)相关性胃炎治疗效果的评价作用.方法 对312例Hp相关性胃疾病患者予以常规克拉霉素、阿莫西林、奥美拉唑三联治疗,8周后采用免疫比浊分析法测定血清PG I、Ⅱ,PCR法测定Hp-DNA的含量变化,联合判断Hp根除效果；分别于治疗前和治疗后4周、8周采用免疫比浊分析法测定血清PG I、Ⅱ的含量,比较Hp组和除菌失败组血清PG的含量.结果 Hp除茵成功者121例,成功率38.78％.与治疗前比较,除茵成功组的PG I、Ⅱ含量明显降低,PG I/PGⅡ比值明显升高:失败组PGI、Ⅱ的变化不明显.结论 根除Hp治疗前后血清PG发生特异性改变,血清PG含量可作为Hp根除效果的判断指标.

Evaluation role of change in serum pepsinogen concentration on treatment effects of Hp-related gastritis

Objective To explore the evaluation role of the concentration change of serum pepsinogen(PGⅠ,Ⅱ) on the treatment effect of Helicobacter pylori(Hp) related gastritis.Methods 312 patients with Hp-related gastritis were treated with triple therapy of clarithromycin,amoxilin and omeprazole.After 8-week treatment,serum pepsinogen was detected by the immune turbidimetry and the concentration change of Hp-DNA was detected by PCR for the combination judgment of Hp eradication effects.The serum pepsinogen concentration was detected by the immune turbidimetry before treatment and in 4,8 weeks after treatment for comparing the serum pepsinogen concentration between the eradication group and non-eradication group.Results 121 cases succeeded in eradication with the success rate of 38.78%.Compared with before treatment,serum PGⅠ,Ⅱ concentrations were significantly decreased and the PGⅠ/PGⅡ ratio was increased significantly in the eradication group.There were no remarkable changes with serum PG concentrations in the non-eradication group.Conclusion Serum PG has specific changes before and after eradication.Serum PG concentrations can be used as the judgment index of Hp eradication.
