产科失血性休克53例治疗体会

目的总结结扎双侧子宫动脉上行支止血治疗产后失血性休克的临床经验。方法将本院1992年1月~2002年1月收治的24例产后失血性休克产妇纳入对照组,以子宫切除为主要止血方法;而2002年2月~2012年2月收治的29例纳入研究组,以结扎双侧子宫动脉上行支为主要止血方法。比较2组产后失血性休克的病因,围产儿及产妇结局的差异。结果 2组在产后失血性休克的病因、围产儿结局、孕产妇死亡率等方面比较,差异无统计学意义(P>0.05),但研究组子宫切除率(10.35%)显著低于对照组(91.67%),差异有统计学意义(P<0.01)。结论结扎双侧子宫动脉上行支止血效果满意,可明显降低子宫切除率。

Treatment experiences with hemorrhagic shock in 53 patients

Objective To summarize the clinical experience of ligating the ascending branch of the bilateral uterine artery for the hemostatic treatment of postpartum hemorrhagic shock.Methods Twenty-four women with hemorrhagic shock who received hysterectomy for hemostasis from January 1992 to February 2012 in our hospital served as control group.Another 29 women who were administered ligation of bilateral uterine artery ascending branch for hemostasis from February 2002 to February 2012 in our hospital were selected as study group.The cause for postpartum hemorrhagic shock and outcomes of perinatal infants and parturients were compared between the two groups.Results The cause and outcomes of perinatal infants and maternal mortality showed no statistical difference between the two groups(P>0.05),but the hysterectomy rate was statistically lower in the study group(10.35%) than in the control group(91.67%)(P<0.01).Conclusion Ligating the ascending branch of the bilateral uterine artery has satisfactory hemostatic effect and can obviously reduce the hysterectomy rate.
