过敏性紫癜合并心脏损害24例临床分析

目的:观察过敏性紫癜患儿合并心脏损害的表现、程度及恢复情况。方法:在收治的75例过敏性紫癜患儿中,对合并心脏损害的24例进行临床分析。所有病例均进行心电图及心肌酶学检查。结果:心电图异常20例,其中过敏性紫癜混合型占15例,单纯型占5例。包括有窦性心动过速、窦性心动过缓、Ι度房室传导阻滞、Q-T间期延长、T波低平或倒置或双向、ST段下移、室性早搏。混合型过敏性紫癜的心肌酶谱高于单纯型过敏性紫癜病例(P<0.01)。而在恢复期心肌酶及心电图均可恢复正常。结论:过敏性紫癜可引起一过性心肌损害,混合型病例更为明显。心电图、心肌酶谱的检查对早期发现过敏性紫癜患儿合并心脏损害有很大的临床意义。

Analysis of 24 cases children with Henoch-Schonlein with myocardial injury

Objective:To investigate the clinical expression,level and recover of Henoch-Schonlein with myocardial injury.Methods:Among 75 cases of children with Henoch-Schonlein,the 24 cases with myocardial injury were analysed.Results:20 patients showed abnormal EKG.Among of them,15 cases were complex Henoch-Schonlein and 5 cases were simple Henoch-Schonlein.The abnormal EKG including sinus tachycardia,sinus bradycardia,atrioventricular block,long Q-T interval,T wave low level,invert or double direction,ST segment falling down,ventricular premature beat.The myocardium enzyme table of complex Henoch-Schonlein was higher than that of simple Henoch-Schonlein(P<0.01).But in the recovering period,myocardium enzyme and EKG could be turned into normal.Conclusion:Henoch-Schonlein can cause an instant myocardial injury,obviously in complex Henoch-Schonlein.EKG and myocardium enzyme table are very important to find the cases of Henoch-Schonlein with myocardial injury earlier.
