摘    要:目的:研究应用弹性测量仪和超声波测量评价烧伤后增生瘢痕的变化。方法:自1995年1月起,长期随访18例随机选择的烧伤患者(年龄为1-60岁),应用皮肤弹性测量仪及超声波影像对患者每月测量一次皮肤在负压吸引作用下的最大变形度(R0)和粘弹性指数(R8),比较增生瘢痕与身体对侧相应部位测量的区别,并将检查结果与传统临床分度方法进行比较分析。同时也对皮肤弹性值与临床观察的瘢痕硬度进行相关性分析。结果:烧伤后增生瘢痕的R8>0.193时,临床硬度分度也较高(9%为0度,14%为1度,30%为2度,46%为3度,83%为4度)。结论:R8>1.93被定为预后数值,大于该数值者需要密切随访。

Abstract:Objective This paper aims at studying the use of elastometry and ultrasonography in recording the progress of post burn hypertrophic scar, and the reliability of using a skin elastometry (Cutometer SEM 575) in the assessment of hypertrophic scar. Methods Eighteen consecutive burn cases were randomly selected and longitudinally studied since January, 1995. Both adults and children with age ranged from 1 to 60 years were recruited. Modes of injury included scald, flame burn, melted wax, cold and frictional burns respectively. The data R0 (total deformation of skin due to the suction), and R8 (the visco elastic character) of the hypertrophic scar were taken and were compared with that obtained from the corresponding normal skin on the contralateral side of the body. The 18 cases were fo llowed up monthly and data were analyzed. The results of the clinical measurement were compared with the traditional grading. A correlation of the skin elastometry with observational firmness was also analyzed. ResultsThere is a higher tendency of post burn hypertrophic scar with R8>0.193 in scars with higher score in clinical grading (9% for grade 0, 14% for grade 1, 30% for grade 2, 46% for grade 3, and 83% for grade 4). Conclusion R8 > 1.93 was recommended as a predictive value to distinguish the scar that needs intensive follow up.
