MR扩散加权磁共振成像在胶质瘤诊断的应用

摘    要:目的探讨MR扩散加权磁共振成像(DWI)在诊断胶质瘤中的价值,为临床正确诊断胶质瘤和最佳治疗方案的选择提供更加可靠的影像学依据。方法采用美国GE公司1.5TMR磁共振成像系统,50例经手术病理证实的胶质瘤病例,均行常规MR平扫、DWI扫描与增强扫描。结果采取病例中,50例胶质瘤,10例脑脓肿,8例脑转移瘤;36例脑胶质瘤患者均为低信号,10例脑脓肿均为高信号,8例脑转移瘤为均匀高信号。脑脓肿与脑胶质瘤的坏死区平均ADC值之间差异有统计学意义(P〈0.01),脑转移瘤与脑脓肿平均ADC值之间差异无统计学意义(P〉0.05)。结论MR扩散加权磁共振成像在胶质瘤诊断价值高,但对于脑转移瘤与脑脓肿的判断还有一定局限性,需要结合多种技术来提高诊断的准确率。

Abstract:Objective To explore value of Diffusion--Weighted MR Imaging (DWI) in diagnosis of glioma so as to offer more reliable imaging ground for correct clinical diagnosis of glioma and selection of optimal treatment scheme.Methods USA GE 1.5TM R magnetic resonance imaging scanner was utilized in conventional plain scanning,DWI scanning and enhanced scanning of 50 patients with glioma confirmed pathologically.Results Among all the taken cases(50 patients with glioma,10 cases with pyenecphalus, 8 patients with metastatic encephaloma) there are 36 patients with glioma showing a low signal,10 patients with pyencephalus showing a high signal and 8 patients with metastatic encephaloma showing a homogeneous high signal. Patients with pyenecphalus and glioma showed a difference of statistical significance in average ADC values (P〈0.01);however, patients with metastatic encephaloma and pyencephalus showed no difference of statisti- cal significance in average ADC values (P〉0.05).Conclusion Diffusion--Weighted MR Imaging (DWI) has a high value in diagnosis of glioma;but with limitation to diagnosis of metastatic encephaloma and pyencephalus, various technologies shall be combined to improve accuracy rate of diagnosis.
