紫杉醇联合卡铂治疗45例复发转移性乳腺癌

背景与目的：晚期乳腺癌的治疗非常困难，疗效较差，寻找有效的联合化疗方案是提高其疗效的关键。本研究的目的是观察紫杉醇联合卡铂方案治疗复发和转移性乳腺癌的疗效与安全性。方法：2001年1月-2006年3月以此方案治疗晚期乳腺癌45例。中位化疗周期数3周期（2—6周期）。结果：可评价疗效45例，CR3例（6．7％），PR25例（55．6％），SD11例（24．4％），PD6例（13．3％），总有效率（CR＋PR）62．2％，中位肿瘤进展时间（TTP）7个月，中位生存期为29个月。1年生存率73．3％。紫杉醇加卡铂方案一线和二线治疗进展期乳腺癌近期疗效分别为62．1％和62．5％。内脏转移与软组织转移之间疗效无明显差别。主要毒性为恶心、呕吐、神经毒性、骨髓抑制。结论：紫杉醇联合卡铂方案治疗复发和转移性乳腺癌疗效较好，使用方便，毒性反应较轻，是晚期乳腺癌的有效解救治疗方案。

Paclitaxel and carboplatin for the treatment of advanced breast cancer

Background and purpose:To evaluate the efficacy and safety of combination chemotherapy of paclitaxel(PTX) and carboplatin(CBP) for advanced breast cancer(ABC). Methods:From January 2001 to March 2006, forty five patients with ABC were treated with combination chemotherapy of PTX and CBP. PTX 175mg/m 2 day 1 every 3 weeks or 75 mg/m 2 weekly, 3 times for every 4 weeks. CBP was administrated on day 2 with dose of AUC 5. The median number of cycles was 3 (range 2~6).Results:The overall response rate was 62.2%, median time to progression was 7 months (95%CI 5.1-8.9). Median overall survival was 29.0 months. 1 year survival rate was 73.3%. Response rate for first line and second line treatment were 62.1% and 62.5%. No significant response difference existed between visceral metastasis and soft tissue metastasis. The main side effect included nausea, vomiting, neurotoxicity and hematologic toxicities. Grade 3 to 4 clinical adverse effect was nausea and vomiting in 2 cases (4.4%),leukopenia in 17cases (37.8%).Conclusions:PTX and CBP combination is active in ABC with an acceptable toxicity pattern.
