连续性血液滤过治疗急性呼吸窘迫综合征的临床评价

目的探讨连续性静脉-静脉血液滤过(CVVH)治疗急性呼吸窘迫综合征(ARDS)的临床疗效。方法回顾性分析64例ARDS患者,分为对照组26例,采用常规治疗 呼吸末正压机械通气(PEEP);CVVH组38例,采用常规治疗 呼吸末正压机械通气(PEEP) CVVH。分别观察2组患者在治疗前后动脉血气指标及PEEP水平的变化。结果38例ARDS患者加用CVVH治疗后,各项血气指标、有效治愈率和死亡率与对照组比较,差异有统计学意义(P<0.05)。结论连续性血液滤过治疗ARDS时能有效清除患者血管外肺水和致炎介质,明显改善了患者的氧合功能,提高了抢救成功率。

Treatment of acute respiratory distress syndrome with continuous hemofiltration

Objective To explore the clinical effects of continuous vena-venous hemofiltration(CVVH) on the treatment of Acute Respiratory Distress Syndrome(ARDS).Method 64 patients in the present study were allocated either into the treatment group or the control group,The treatment group consisted of 38 patients who received routine therapy mechanical ventilation CVVH.The control group consisted of 26 patients who received only routine therapy mechanical ventilation.Difference between the treatment and the control groups in arterial blood gas index and PEEP were observed.Results After CVVH therapy under mechanical ventilation,patients in the treatment group had significant increase in arterial blood gas index and the effective rate,and significant decrease in mortality rate when compared with those in the control group.Conclusion CVVH therapy could effectively remove extravascular lung water and proinflammatory mediators and markedly improve oxygenation of patients with ARDS,therefore increase the success rate of resuscitation.
