临时起搏器在AMI慢性心律失常中的应用研究

目的探讨心脏临时起搏器治疗急性心肌梗死（AMI）慢性心律失常的临床应用价值。方法我科收治AMI合并慢性心律失常患者30例,植入临时起搏器后,观察血压改变、治疗效果及术后并发症等情况。结果起搏器植入后患者收缩压、舒张压及平均脉压差较植入前明显升高（P〈0.01）;在临时起搏器的支持下临床症状明显改善,抢救成功率90%;平均植入时间（10.5±2.5）min;平均起搏时间（5.2±3.5）d;经临时起搏后,恢复窦性心律,顺利渡过超急期,仅1例需行永久性起搏器植入治疗;未出现严重并发症,予相应处理后症状均消失。结论临时起搏器在AMI慢性心律失常的治疗中效果肯定,能有效改善血流动力学,抢救成功率高,安全有效。

Application of artificial temporary pacemaker machine in acute myocardial infarction with chronic arrhythmia

Objective To investigate the clinical application of artificial temporary cardiac pacemaker in the treatment of acute myocardial infarction(AMI) with chronic arrhythmia.Methods A retrospective analysis of 30 cases of AMI with chronic arrhythmia after temporary pacemaker implantation was made,and the hemodynamic alterations,therapeutic effect and complications were observed.Results The hemodynamic alterations significantly improved compared with that prior to implantation,with statistical significance(P<0.01).With temporary pacemaker support,the clinical symptoms improved,the successful rescue rate was 90%;the implantation average time was(10.5±2.5)min;average pacing time was(5.2±3.5)d;only one patient required permanent pacing implant treatment and no severe complications occurred.Conclusion Artificial temporary cardiac pacemaker is effective in the treatment of AMI with chronic arrhythmia.It can effectively improve hemodynamics and rescue rate.
