仙灵骨葆胶囊对骨质疏松症患者细胞因子及骨代谢、骨转换指标的影响

摘    要:目的探讨仙灵骨葆胶囊对骨质疏松症(OP)患者细胞因子及骨代谢、骨转换指标的影响。方法选取2015年10月-2016年9月收治的OP患者68例,随机分为观察组及对照组,各34例,对照组口服阿仑膦酸钠和碳酸钙D3,观察组在对照组基础上口服仙灵骨葆胶囊。于治疗前及治疗后3、6个月测量右股骨颈、腰椎(L2~4)正位骨密度(BMD);于治疗前及治疗后1、2个月采用放免法测定肿瘤坏死因子-α(TNF-α)、胰岛素样生长因子-1(IGF-1)、白细胞介素-6(IL-6);于治疗前及治疗后3、6个月采用电化学发光免疫分析法测定血清骨钙素(BGP),ELISA法测定Ⅰ型胶原交联C-末端肽(CTX-1)、核因子кB受体活化因子配基(RANKL)、骨保护素(OPG)。结果治疗后6个月观察组右股骨颈、L2~4正位BMD值明显高于对照组(P0.05);观察组治疗后1、2个月TNF-α、IL-6低于同期对照组,IGF-1高于同期对照组(P0.05,P0.01);观察组治疗后3、6个月BGP、OPG高于同期对照组,CTX-1、RANKL低于同期对照组(P0.05,P0.01)。结论常规抗骨质疏松药物基础上联合仙灵骨葆胶囊可有效增加OP患者BMD,抑制细胞因子的表达,调节骨代谢及骨转换状态,从而改善患者临床症状。

Abstract:Objective To explore the effect of Xianling Gubao capsule on the cytokines,bone metabolism,and bone turnover indicators in patients with osteoporosis (OP). Methods A total of 68 patients with OP who were admitted in our hospital from October, 2015 to September, 2016 and randomized into the observation group and the control group with 34 cases in each group. The patients in the control group were orally administrated with alendronate sodium and calcium carbonate D3.On the above basis,the patients in the observation group were given Xianling Gubao capsule.BMD of right femoral neck and anterior position of lumbar spine(L2~4)before treatment,3 and 6 months after treatment in the two groups was measured. The radioimmunoassay was used to detect TNF-α, IGF-1, and IL-6 before treatment, 1 and 2 months after treatment. The electrochemiluminescence immunoassay was used to detect the serum BGP before treatment, 3 and 6 months after treatment. ELISA was used to detect CTX-1, RANKL, and OPG. Results BMD of right femoral neck and anterior position of lumbar spine (L2~4) 6 months after treatment in the observation group was significantly higher than that in the control group (P<0.05). TNF-α and IL-6 levels 1 and 2 months after treatment in the observation group were significantly lower than those in the control group, while IGF-1 was significantly higher than that in the control group (P<0.05, P<0.01). BGP and OPG 3 and 6 months after treatment in the observation group were significantly higher than those in the control group, while CTX-1 and RANKL were significantly lower than those in the control group (P< 0.05, P< 0.01). Conclusions Routine anti-osteoporosis drugs in combined with Xianling Gubao capsule can effectively increase BMD in patients with OP, inhibit the expressions of cytokines, and regulate the bone metabolism and bone turnover status in order to improve the clinical symptoms.
