口服降糖药失效的老年2型糖尿病患者应用地特胰岛素联合阿卡波糖治疗的效果观察

目的：分析地特胰岛素联合阿卡波糖治疗口服降糖药失效的老年2型糖尿病患者的效果。方法选取口服降糖药失效的80例老年2型糖尿病患者，随机分为两组，每组40例，试验组给予地特胰岛素联合阿卡波糖，对照组给予诺和灵。结果对照组和试验组用药4、8和12周后的FBG和2 hBG均较治疗前降低，差异有统计学意义（P＜0．05）。两组治疗12周后HbA1 c分别较治疗前降低，差异有统计学意义（P＜0．05）。试验组治疗12周后FC-P的明显高于治疗前，差异有统计学意义（P＜0．05），2 hC-P较治疗前明显升高，差异有统计学意义（P＜0．05）。试验组胰岛素用药剂量、低血糖发生率明显低于对照组，差异有统计学意义（P＜0．05）。结论地特胰岛素联合阿卡波糖治疗口服降糖药失效的老年2型糖尿病，可取得良好临床疗效，减少低血糖发生。

Effect observation of insulin detemir combined with acarbose treated elderly patients with type 2 diabetes after oral failure of hypoglycemic drugs

Objective To analyze the effect of insulin detemir combined acarbose treatment with oral hypoglycemic agents failure in elderly patients with type 2 diabetes.Methods Eighty elderly patients with type 2 diabetes after oral failure of oral hypoglycemic agents were randomly divided into two groups with 40 cases in each group;the experimental group received insulin detemir joint acarbose,and the control group received novolin.Results After 4,8 and 12 weeks treatment of two groups,the FBG and 2hBG were reduced,and compared with the prior treatment,the difference was statistically significant （P〈0.05）.The HbA1c was（7.54 ±0.43）and（7.61 ±1.25）respectively after 12 weeks treatment,lower than before treatment,which was（9.14 ±1.17）and（9.29 ±1.27）,and the difference was statistically significant （P〈0.05）.The FC-P was significantly higher than that before treatment in experimental group after 12 weeks treatment.The results of 2hC-P（3.38 ±0.27）increased significantly compared with those of prior treatment（2.41 ±0.15）（P〈0.05）.Insulin dose and the incidence of hypoglycemia of experimental group were significantly lower than those of the control group,and the difference was significant （P〈0.05 ）. Conclusion Detemir combined with oral hypoglycemic agents acarbose in the treatment of type 2 diabetes after oral failure of hypoglycemic drugs can achieve good clinical efficacy and reduce the incidence of hypoglycemia.
