VEGF、CD34在评价宫颈癌动脉化疗后肿瘤血管生成作用的临床研究

摘    要:目的研究宫颈癌组织中血管内皮生长因子(VEGF)、CD34微血管密度(MVD)与新辅助动脉化疗临床效果的关系.方法随机选择60例宫颈癌患者(动脉化疗组32例,静脉化疗组28例)进行研究,采用免疫组化(IH—CA)、链霉抗生物素蛋白-过氧化酶sP法检测化疗前和化疗后15d宫颈癌组织血管生成的相关因子VEGF及CD34微血管密度(MVD),并进行病理效果的评价.结果动脉化疗组的病理缓解率为81%,与静脉化疗组的53%比较,差异有统计学意义(P〈0.05);通过对动脉化疗前、后肿瘤血管生成因子的检测,发现治疗前组织中CD34微血管密度(MVD)增高者,其病理缓解疗效明显优于微血管密度低者,而静脉化疗前后相关因子表达的比较差异无统计学意义.结论动脉化疗的疗效优于静脉化疗,CD34微血管密度(MVD)的检测可作为预测患者动脉化疗敏感性的指标之一.

Abstract:Objectives To research the relationship among vascular endothelial growth factor (VEGF), the CD34 microvessel density (MVD) and the effect of properative arterial interventional neoadjuvant chemotherapy in cervical cancer. Methods We selected randomly 60 cases of cervical cancer (32 cases of arterial inter- ventional chemotherapy, 28 cases of intravenous chemotherapy) , the expression of VEGF and CD34 MVD factors were examined before and after 15 days of chemotherapy by immunohistochemical-SP. Results The overall re- sponse rate of arterial interventional chemotherapy (81%) was higher than that of intravenous chemotherapy (53%) ; By the detection before and after arterial interventional chemotherapy, the tissues with higher CD34 MVD had the better clinic effect and pathologic results, and there was no difference of the expression before and after ntravenous chemotherapy in the tissues. Conclusion The effect of arterial interventional chemotherapyt is better than that of intravenous chemotherapy, and the CD34 MVD can be one of the indexes to predict the sensitivity of arterial interventional chemotherapy.
