三孔法腹腔镜下胆总管探查与T管引流术70例

目的探讨三孔法腹腔镜胆总管探查与T管引流术在临床上的应用价值。方法70例患者，采用三孔法腹腔镜胆总管探查与T管引流术：即腹腔镜下切开胆总管，通过加长的10mm穿刺器利用纤维胆道镜探查胆总管，合并利用体内冲击波胆道碎石机碎石，取出结石后，行胆总管T管引流。术后50天拔T管，经T管窦道纤维胆道镜探查肝内外胆道情况。结果70例手术成功，无中转开腹，手术时间103．4-36min，术后无胆汁渗漏及其他并发症发生。结论三孔法腹腔镜胆总管探查术操作安全、可靠，有临床应用价值。

Experience of Three-port Laparoscopic Common Bile Duct Exploration and Drainage by T Duct

Objective To investigate the application of three - port laparoscopic common bile duct exploration and drainage by T duct. Methods The common bile duct of 70 case was slited by knife and explored by fabric choledochoscope through three - port laparoscopic. After the stone was taken out by net , the common bile duct was drained by T duet. After 50 days of the operation, the bile duct within and besides liver was explorated by fabric choledochoscope. Results The 35 operations were successfully performed without being converted to open surgery,and the time of the operations was （ 103 ±36 ） minutes, and there was no bile leakage after operation. Conclusions The operation of tree - port laparoscopic common bile duct exploration and drainage by T duct was safe and reliable, and it was worthy of being applicated in clinic.
