头颈部腺样囊性癌的临床病理分析

目的 :分析腺样囊性癌的临床病理特点 ,探讨影响预后的有关因素。方法 :对 1989年～ 1999年连续就诊的 10 9例腺样囊性癌病例进行了回顾性研究 ,对放疗、手术切缘、神经侵犯、病理类型等对预后的影响作了分析。结果 :全部病例中复发 37例 ,远处转移 2 9例 ,2 5例死于本病 ,5例失访 ,总体 5年生存率为 63.4 %。放疗病例与未放疗病例的 5年生存率差异无显著性 ;实性型与其他病理类型的 5年生存率差异也无显著性 ;切缘阳性与阴性及有无沿神经侵犯病例的 5年生存率差异均有显著性。结论 :影响腺样囊性癌预后的主要临床病理因素是切缘阳性及沿神经生长。

Clinicopathologic Study of Head Neck Adenoid Cystic Carcinoma

Objective To make a clinicopathologic study of head and neck adenoid cystic carcinoma and determine the factors influencing survival.Methods A retrospective study was made on 109 cases of head and neck adenoid cystic carcinoma.Among all cases there were 73 males and 36 females.The average age was 51 5 years.The factors including radiotherapy,surgical margin,perineural invasion and pathologic pattern were studied.Results Of the 109 cases 37 developed local recurrences,29 developed distant metastases,25 died of disease,5 missing.The overall 5 year survival rate was 63 4%.The survival rate in patients with positive surgical margin and perineural invasion had statistic significant difference to those with none.Wheras the use of postoperative radiotherapy had no statistic significant advantage in survival.Conclusions Adenoid cystic carcinoma had a long term survival.Positive surgical margin and perineural invasion associated with poor survival.
