应用牙冠延长术修复前牙残根的美学评价

目的观察牙冠延长术在前牙残根修复中的临床效果。方法选择本院2007。2010年25例患者68颗牙体缺损达龈下的残根，采用牙冠延长术暴露龈下牙体断面，术后6～8周行桩冠修复。记录手术前后残根的菌斑指数（PLI）、牙周探诊深度（PD）、龈沟出血指数（SBI）、牙齿松动度及修复效果、Kay牙齿美学效果评价，观察临床疗效。结果所有病例在术后1周牙龈有不同程度炎症表现，在术后6周牙体断端均暴露较好，牙龈正常。PD、SBI均明显优于术前（P〈0．05），经冠修复后残根残冠无松动，修复效果和美学效果满意，美观效果均达Kay牙齿美学分类ClassⅡ-Ⅰ（化妆）或ClassⅡ-Ⅰ标准。结论牙冠延长术有利于残根残冠的保存和修复，并有利于修复后牙龈的美观和健康。

Clinical esthetics evaluation on anterior residual roots restoration using surgical crown lengthening

Objective To observe the clinical esthetics evaluation on residual roots using surgical crown lengthening.Methods A total of 25 patients’ 68 residual subgingival teeth were treated with surgical crown lengthening(SCL),then restored with post-core crown after 6-8 weeks.Before and after the treatment,the PLI,PD,SBI were used to access the gingival condition,restoration results were evaluated by use of Kay’s dental esthetics classification.Results All the patients occured inflammation 1 week after SCL,but after 6 weeks the gingival health was significantly improved.All the cases were up to Kay’s dental esthetics classification Class Ⅱ-Ⅰ(cosmetic) or Class Ⅱ-Ⅰ criteria.Conclusion SCL is a good method to keep the residual roots and contribute to the satisfactory esthetic restoration.
