摘    要:目的:分析口腔癌患者根治手术前、后营养状况改变,探讨营养支持对其康复的临床价值。方法:收集30例口腔癌患者为研究对象,询问病史,分别于手术前1天、术后第1天、术后第7天评价其进食及体重变化情况,测量身高、体重并测定血红蛋白、淋巴细胞计数、血清白蛋白和前蛋白。采用SPSS11.5软件包对结果进行t检验和χ2检验。结果:①膳食调查,术后蛋白质、热能的摄入量显著低于术前(P<0.01);②蛋白指标测定,术后第1天、第7天的血红蛋白含量显著低于术前(P<0.01),术后血清白蛋白含量较术前显著降低(P<0.01),血清前白蛋白含量术后虽比术前下降,但无显著差异;③人体测量,上臂肌围值在术后第1天低于术前,但无统计学差异,而术后第7天则显著低于术前(P<0.05);④免疫功能测定,总淋巴细胞计数术后第1天、第7天显著低于术前(P<0.01)。结论:口腔癌手术及术后进食障碍使患者营养状况指标明显降低。

Abstract:PURPOSE: To analyze the difference of nutrition condition in patients with oral cancer before and after radical surgery,and evaluate the role of nutrition support in oral cancer patients rehabilitation.METHODS: Thirty patients with oral cancer after radical surgery were comprised of this group.The medical history,the intake condition,body weight changes and height 1 day before surgery,1 day and 7 days after surgery were recorded and assessed.The serum hemoglobin(Hb),lymphocyte count(LC),albumin(ALB),pre-albumin(PA) of the patients were detected.SPSS11.5 software package was used for Student’s t test and Chi-square test.RESULTS: ①Dietary survey showed that protein,energy intake was significantly lower than after operation(P<0.01);②Hemoglobin was significantly lower 1day,7 days,preoperative(P<0.01);serum albumin decreased significantly after surgery(P <0.01);Serum pre-albumin decreased after surgery,the difference was not significant;③The value of upper arm muscle circumference decrease 1 day after surgery,but didn’t reach statistical significance.7 days after surgery,the difference was significant(P <0.05);④Total lymphocyte count 1 day,7 days after operation was significantly lower than 1 day before surgery(P <0.01).CONCLUSION: Radical surgery of oral cancer and eating disorders decreased nutritional status of patients and nutritional support is mandatory.
