保肝护脑针法治疗脑缺血再灌注损伤的机制研究

摘    要:目的探讨保肝护脑针法对脑缺血再灌注大鼠的治疗作用。方法将SD雄性大鼠随机分为假手术组、模型组、针刺对照组、保肝护脑针法组。线栓法制作缺血再灌注大鼠模型,测试大鼠行为学、血清谷草转氨酶(AST)、血清还原型谷胱甘肽(GSH)和血清总抗氧化能力(T-AOC),并观察肝细胞线粒体形态变化。结果模型组、针刺对照组和保肝护脑针法组缺血2 h(电针治疗前)时神经功能缺损评分无显著差异;经10次电针治疗结束后:与模型组相比,针刺对照组和保肝护脑针法组神经功能缺损评分有极显著差异(P<0.01);与针刺对照组相比,保肝护脑针法组神经功能缺损评分变化更为显著(P<0.05)。电针治疗前,与假手术组、针刺对照组和保肝护脑针法组比较,模型组血清AST水平显著增高(P<0.01),而血清GSH和T-AOC明显降低(P<0.01);经10次电针治疗结束后:与模型组相比,针刺对照组和保肝护脑针法组血清AST水平明显降低(P<0.01),同时血清GSH和T-AOC显著升高(P<0.01);与针刺对照组相比,保肝护脑针法组血清AST、GSH和T-AOC各项变化更为显著(P<0.05)。电镜观察肝细胞线粒体形态改变:假手术组肝细胞线粒体形态正常,模型组肝细胞线粒体明显肿胀,针刺对照组肝细胞线粒体轻度肿胀,保肝护脑针法组肝细胞线粒体形态近乎正常。结论保肝护脑针法对脑缺血再灌注大鼠的治疗效果较单纯头穴针刺更为显著,其治疗机理与抗氧应激密切相关。

Abstract:Objective To explore the effect of protecting liver to nourish brain therapy on cerebral ischemic reperfusion(CIR)rats.Methods The 40 Sprague Dawley rats were randomly divided into sham operation,CIR model,acupuncture control,protecting liver to nourish brain therapy(PLNB) groups,each with 10 rats.CIR models were established by right middle cerebral artery occlusion(MCAO).The neurological deficit scores were evaluated.The serum aspartate amino transferase(AST),glutathione(GSH),total antioxidant capacity(T-AOC)were detected separately by biochemical methods.The mitochondria of liver was observed by electron microscopy.Results Before treatment,the neurological deficit scores were no difference among CIR model,acupuncture control,PLNB groups.After treatment,in comparison with CIR model group,significant differences of the neurological deficit scores were found in acupuncture control and PLNB groups(P<0.01).there was more significant changes of the neurological deficit scores in observe group than acupuncture control one(P<0.05).Before treatment,in comparison with sham operation,acupuncture control,PLNB groups,the level of serum AST was higher but the level of serum GSH and T-AOC lower in CIR model group(P<0.01).After treatment,in comparison with CIR model group,significant differences of the level of serum AST、GSH、T-AOC were found in acupuncture control and PLNB groups(P<0.01).There was more significant changes of serum AST、GSH、T-AOC in PLNB group than acupuncture control one(P<0.05)Under observation of electron microscopy,the mitochondria of liver.was highly swelling in model group,the mitochondria of liver was nearly normal in PLNB group.Conclusions The protecting liver to nourish brain therapy had more effects on CIR rats.than simple scalp needling,its therapeutic effects closely related to anti-oxygen stress.
