远位带蒂皮瓣修复手外伤术后肩关节并发症的护理

目的探讨远位带蒂皮瓣修复手外伤术后早期进行术侧肩关节活动训练、推拿及远红外线照射对术后肩关节并发症的预防效果。方法将260位远位皮瓣修复手外伤的患者分为对照组130例及观察组130例。观察组在术后第1天开始行肩关节主动训练、局部推拿和远红外线治疗；对照组按常规要求患者术后在防止皮瓣撕脱的前提下保持肩关节的适当活动，活动强度、时间未做要求，量力而行。观察二期断蒂术后两组患者肩关节并发症发生情况。结果二期断蒂术后，观察组发生术后肩关节疼痛、功能受损6例，对照组发生69例，两组比较差异有统计学意义（P〈0．01）。结论术后早期肩关节主动训练、局部推拿及远红外线照射能有效预防远位带蒂皮瓣修复手外伤术后肩关节疼痛、功能受损的发生。
