分化型甲状腺癌的外科治疗

摘要目的探讨分化型甲状腺癌的外科治疗方式。方法回顾性分析86例分化型甲状腺癌的临床资料及随访结果。结果采取一侧甲状腺叶加峡部切除45例，颈淋巴结清扫联合根治术38例，姑息性治疗3例。本组10年内死亡12人，5例死于肿瘤复发或转移，另7例属非肿瘤死亡，即10年总死亡率14．0％，癌死亡率5．81％，生存率为86％，实际10年生存率为94．19％。结论对分化型甲状腺癌，主张行惠侧腺叶加峡部切除。对颈淋巴结阳性患者，同时行惠侧功能性颈淋巴结清扫术；而颈淋巴结阴性患者不必行颈清扫术，但提倡长期密切随访。

The Surgical Treatment for Differerntiated Thyroid Carcinoma

Objective To study the type surgical treatment method in the differentiated thyroid carcinoma ( DTC).Method The clinical data and follow-up results of 86 cases of DTC were retrospectively analysed . Result Conservative thyroid lobectomy +isthmectomy excision 16 cases; Radical neck dissection 38 cases; The appeasement cures 3 cases.This 10 yearses inside dies 12 people,5 examples die in the tumor relapses or transfers, another 7 examples belong to not tumor death;Then total death rate in 10 years 14.0%, cancer death rate 5.81%, exist rate as 86%, actual 10 yearses exist rate as 94.19%.Conclusion We recommend lobectomy+ isthmectomy for tumor side for most of DTC. Selective neck dissection is not necessary for No cases.
