复杂性囊性肾脏占位病变的诊治体会

目的：探讨复杂性囊性肾脏占位病变的诊断和治疗。方法：对我院1997年6月～2007年11月收治的37例复杂性囊性肾脏占位病变患者临床资料进行回顾性分析。结果：37例复杂性囊性肾脏，发现肾脏占位病变43个。B超以低回声为主病变13个，等回声病变为主17个，高回声病变为主13个；CT诊断Ⅱ类囊性病变24例（26个），Ⅲ类囊性病变8例（12个病变），Ⅳ类囊性病变5例（5个病变）；手术18例，其中4例为肾脏恶性肿瘤。随访3～6年，死亡2例，均为恶性肿瘤晚期全身转移。结论：复杂性囊性肾脏占位病变容易出现误诊，B超检查结合CT扫描是其主要诊断方法，Ⅲ、Ⅳ类囊性病变均应手术治疗。

Diagnosis and management of complicated cystic renal occupying lesions

Objective: To discuss diagnosis and management of complicated cystic renal occupying lesions.Methods:Retrospective study was conducted on 37cases of complicated cystic renal occupying lesions from Jun,1997to Nov,2007.Results:43 cystic renal occupying lesions were found in 37 patients,According to ultrasound diagnostic criteria,13were hypoechogenic,17were isoechongnic and13 were hyperechogenic.Category of these lesions under CT examination was based on the Bosniak system.26lesions of class IIwere found in 24patients,12 lesions of class IIIin 8 patients and 5 lesions of class IV in 5patients.Open operation was done on 18 cases including 4 cases of renal carcinoma.2patients died from tumor metastasis during following-up.Conclusions:Misdiagnosis of complicated cystic renal occuping lesions is mot rare.Ultrasound in combination with CT is reliable diagnostic method for such leson,It' s reasonable for patients with class III or class IV lesions to accept operation
