强直性脊柱炎患者中医证型与生存质量关系的研究

目的:探讨强直性脊柱炎(AS)患者中医证型与生存质量的关系。方法:采集142例AS患者人口学资料、中医证候、SF-36量表及实验室指标等临床评价指标进行相关统计,分析肾虚督寒型、肾虚湿热型及肝肾不足型三种证型与生存质量关系,同时研究中医证型与AS评价指数之间的相关性。结果:肾虚督寒型患者在本次调查中生存质量总分最低(P<0.05),突出表现在活力及精神健康两个维度(P<0.05);不同证型AS患者生存质量生理功能、生理职能、躯体疼痛、活力、精神健康等维度上均有不同(P<0.05);不同证型AS患者BASDI、BASFI评分及炎性指标CRP、ESR间存在差异(P<0.05)。结论:不同证型AS患者生存质量受损维度不同,AS临床评价指数可以在一定程度上反映不同证型的特点。在辨证施治的过程中采取针对性的措施更有利于提高中医药的疗效,改善患者生存质量。

Study on Quality of Life of Ankylosing Spondylitis with Different Syndrome Types

Objective：To investigate the relation of TCM syndromes and survival of ankylosing spondylitis （AS） patients. Meth- ods：The demographic data, syndromes, SF- 36 scale clinical evaluation and laboratory indicators relevant statistical indicators of 142 AS patients were collected. The relation of kidney deficiency and Du meridian cold type, kidney deficiency and damp-heat and liver and kidney deficiency type and life quality was analyzed and the correlation between the index and syndromes was evaluated. Results ： Patients with kidney deficiency and Du meridian cold type in this survey had the lowest score of quality （ P 〈 0.05 ）, es- pecially in vitality and mental health （ P 〈 0.05 ）. The physiological functions, physical function, bodily pain, vitality, mental health and other dimensions were different in different syndromes （ P 〈 0.05 ）. AS BASDI, BASFI scores and inflammatory mark- ers CRP, ESR had significant differences among different sydnromes （ P 〈 O. 05 ）. Conclusion ： AS patients with different syndromes have different impaired quality of life. VAS scores and physical function in patients can reflect the BASFI. The CRP and ESR are important for evaluating the quality of life in patients with AS. In syndrome differentiation process, taking targeted measures can help to improve the efficacy of Chinese medicine the patients＇quality of life.
