中药保留灌肠治疗慢性肾功能衰竭的Meta分析

目的:评价中药灌肠治疗慢性肾功能衰竭(CRF)的有效性和安全性,为临床上优化CRF治疗方案提供强有力的证据。方法:检索国内各种医学期刊,选择在西医常规治疗基础之上加用中药灌肠与单纯西医常规治疗CRF的临床RCT文献。评价纳入研究文献的质量,并进行Meta分析。结果:经严格筛选,最终纳入文献13篇,涉及1003例患者,但其方法学质量均较低。与对照组(西医常规治疗)比较,治疗组(中药灌肠+西医常规治疗)治疗CRF在总有效率、降低BUN、UA、改善恶心呕吐方面有统计学意义,在降低Scr、升高Hb、Ccr、改善纳差方面无统计学意义;纳入文献均无不良反应报道。结论:中药灌肠治疗CRF有一定的疗效,但还需要设计良好的随机对照及多中心临床试验做进一步证实。不良反应方面亦有待于今后的临床研究及毒理学研究提供依据。

Meta-analysis of TCM Retention Enema in Treatment of Chronic Renal Failure

Objective ： To evaluate the effect and safety of TCM enema in the treatment of chronic renal failure, and to provide strong evidence for the optimization of CRF clinical treatment programs. Methods ： Domestic medical journals were reviewed, and we selected the RCT documents which were conventional treatment of Western medicine combined with TCM enema and single routine treatment. The quality of research was evaluated, and the Meta -analysis was carried on. Results：After strict screening, and finally 13 were included, involving 1003 patients, but their methodological quality was low. Compared with the control group （rou- tine treatment） ,the treatment group （conventional treatment of Western medicine and traditional Chinese medicine enema） was statistically significant on CRF efficiency, reducing BUN and UA, and relieving nausea and vomiting and had no significance on el- evating Hb, Scr and Ccr and improving appetite. There was no adverse reaction reported in these documents. Conclusion：TCM ene- ma has certain curative effect in the treatment of CRF, but it also needs to design a good randomized and multi - center clinical trial be confirm. Adverse reaction has to be proved on the basis of clinical research and toxicology research.
