MSCT灌注成像在乳腺肿瘤诊断中的临床应用

摘    要:目的 评价MSCT灌注成像对乳腺良恶性肿瘤的诊断价值.资料与方法 对20例乳腺肿瘤(包括15例乳腺癌和5例纤维腺瘤)的MSCT灌注成像进行回顾性分析.结果 乳腺癌组与对侧正常乳腺组间血流量(BF)、血容量(BV)和表面通透性(PS)差异有统计学意义(P<0.05),但MTT均值差异无统计学意义(P>0.05).纤维腺瘤组与乳腺癌组之间的BF、BV和PS差异有统计学意义(P<0.05),MTT均值差异无统计学意义(P>0.05).大于2 cm的乳腺癌的中心与边缘之间的BF、BV和 PS差异有统计学意义(P<0.05), MTT均值差异无统计学意义(P>0.05).小于2 cm的乳腺癌的中心与边缘之间的BF、BV、MTT和PS差异无统计学意义(P>0.05).结论 MSCT灌注能反映乳腺病变的血液灌注特点,有助于乳腺良、恶性病变的鉴别诊断.

Abstract:Objective To evaluate multi-slice computed tomography (MSCT) perfusion imaging findings of Breast Tumors. Materials and Methods MSCT perfusion imaging findings in 20 cases with breast tumors including 15 breast carcinomas and 5 breast fibroadenomas were reviewed retrospectively. Results There was significant difference in the mean blood flow (BF), blood volume (BV) and permeability surface (PS) between breast carcinomas and normal contra-lateral breast tissues(P<0.05) with no significant difference in the mean transmit time (MTT) between them(P>0.05). There was significant difference in the mean BF, BV, and PS between breast carcinomas and breast fibroadenomas (P<0.05) with no significant difference in MTT between them (P>0.05). There was significant difference in the mean BF, BV, and PS between the center areas and the peripheral areas of breast carcinomas more than 2 centimeters in the diameter (P<0.05) with no significant difference in MTT between them (P>0.05). There was no significant difference in the mean BF, BV, PS, and MTT between the center areas and the peripheral areas of breast carcinomas less than 2 centimeters in the diameter (P<0.05). Conclusion MSCT perfusion imaging could reveal hemodynamics characteristics of breast tumors, contributing to differential diagnosis of benign and malign breast tumors.
