围产儿死亡分析

目的:探讨影响围产儿死亡的主要原因及预防措施。方法:回顾性分析2003~2005年广州开发区围产儿死亡病例的一般情况、死因分布、产前检查和高危妊娠情况,比较不同级别医院围产儿死亡构成比。结果:围产儿死亡的前3位死因依次为畸形、新生儿窒息、脐带因素;围产儿死亡病例中,超过4/5的孕妇没有接受定期产检;高危妊娠使围产儿死亡的风险增加,OR为8.21;二级、一级医院围产儿死亡构成比不同,差异有显著性意义。结论:降低围产儿死亡率必须采取综合措施,从提高人群的健康意识、加强和完善社会医疗保障体系、提高围产期保健质量三方面着手。定期产检、加强高危妊娠监测是降低围产儿死亡率的有力措施,不同级别的医院采取的措施应各有侧重。

Analysis of perinatal death from 2003 to 2005 in Guangzhou

Objective:To investigate the influential factors of perinatal death,and propose the possible interventions.Methods:The perinatal death cases,which had happened in Guangzhou Development Districts from 2003 to 2005,have been selected as objects.Their general status in common,death causes,antepartum examinations,and high-risk pregnancies were analyzed retrospectively.Furthermore,the perinatal mortality in different hospitals at different level,has been compared as well.Results:Malformation,asphyxia of newborns,and umbilic problems were the first three factors to cause the perinatal death.In those perinatal death cases,more than four fifth pregnant women didn't take regular antepartum examinations.Probability of prenatal death had been increased by high-risk pregnancy as well.The OR was 8.21(3.95～17.09).The perinatal death ratios varied significantly between the first and second level of hospitals.Conclusion:Multiple interventions must be put in place to reduce the perinatal mortality by enhancing health education,developing social medical security system,and improving the quality of antenatal care.Regular antepartum examinations and high-risk pregnancy monitoring are the key factors to reduce perinatal mortality.The degree of emphasis of perinatal death interventions should vary in different level of hospitals.
