高频超声与钼靶片对35岁以下女性乳腺癌临床诊断价值的探讨

目的探讨高频超声与钼靶片诊断35岁以下女性乳腺癌的相关临床特征及其意义。方法回顾性分析1999年1月至2006年3月收治的35岁以下女性乳腺癌的高频超声、钼靶片与病理诊断符合率,对诊断乳腺癌的雌孕激素受体状态及C-erbB-2表达进行相关分析。结果35岁以下青年乳腺癌超声诊断与病理诊断符合率为66.7%,钼靶片诊断符合率为68.2%,超声与钼靶片联合诊断符合率为71.1%。在超声、钼靶、联合诊断乳腺癌患者中ER阳性表达率分别为61.5%、61.5%和60.8%,PR阳性表达率分别为59.0%、65.4%和69.6%,C-erbB-2表达率分别为76.5%、87.0%和85.0%。结论超声与钼靶对35岁以下乳腺癌诊断率差异无显著性,联合诊断对提高诊断率意义不大。超声、钼靶诊断率与ER、PR、C-erbB-2阳性表达无显著性相关。

Value of high frequency ultrasonography and mammography for clinical diagnosis of breast cancer

Objective To study the clinical characteristics,significance of high frequency ultrasonography and mammography for diagnosis of breast cancer in women under 35 years of age.Methods From January 1999 to March 2006,the breast cancer patients under 35 years of age underwent preoperative ultrasonography and mammography.The coincidence rate of diagnosis of breast cancer with pathology,ER,PR status and expression of C-erbB-2 was analyzed retrospectively.Results The coincidence rate of diagnosis by ultrasonography with pathology of breast cancer was 66.7%,that by mammography was 68.2% and that by ultrasonography combined with mammography was 71.1%.In patients with breast cancer diagnosed by ultrasonography,mammography and ultrasonography combined with mammography,ER positive rate was 61.5%,61.5% and 60.8%,PR positive rate was 59.0%,65.4% and 69.6%,expression rate of C-erbB-2 was 76.5%,87.0% and 85.0% respectively.Conclusions There is not difference between diagnosing rate of ultrasonography and mammography,and the rate of diagnosis by ultrasonography combined with mammography does not improve.There are no significant correlations between expression of ER,PR,C-erbB-2 and diagnosis by ultrasonography or mammography.
