MSCT在视网膜母细胞瘤经眼动脉化疗前分期的应用

目的:探讨多层螺旋CT在视网膜母细胞瘤经眼动脉化疗前分期的应用价值。方法:回顾性分析30例婴幼儿视网膜母细胞瘤的CT表现,并根据CT分期进行评估,选择病例进行眼动脉灌注化疗。结果:30例38眼表现为眼球内单发肿块34眼,眼内多发肿块3眼,眼内肿块及视神经管累及鞍上1眼。本组病例全部发现单发或多发钙化灶,钙化率为100%。斑点样钙化5眼,斑片状钙化22眼,混合性11眼。Danziger分期Ⅰ期31眼,Ⅱ期6眼,Ⅲ期1眼。Ⅰ期、Ⅱ期病例行眼动脉灌注化疗,Ⅲ期放弃治疗。结论:多层螺旋CT在婴幼儿视网膜母细胞瘤经眼动脉灌注化疗前的定性诊断、肿瘤分期、评价肿瘤侵犯范围及病例选择具有重要价值。

Value of MSCT in staging of retinoblastoma before transcatheter ophthalmic artery chemotherapy

Objective：To investigate the value of multi-slice computed tomography （MSCT） for staging of retinoblas toma before transcatheter ophthalmic artery chemotherapy （TOAC）. Methods：This retrospective study included 38 eyes of 30 patients with retinoblastoma. The CT results of all patients were staged based on Danziger staging and decision would be made which case should be treated with TOAC. Results：There were 34 eyes with single mass and 3 eyes with multiple mas- ses, the other one having masses both in the eye and brain. The calcifications were found in all masses, the rate of calcifica- tion was 100%. There were spotty calcifications in 5 eyes,patchy calcifications in 22 eyes and mixed calcifications in the oth- er 11 eyes. According to Danziger staging,there were 31 eyes with stage I ,6 eyes with stage Ⅱ and 1 eye with stage Ⅲ. Only stage I and stage Ⅱ cases could be treated with TOAC,and the stageⅢ cases not stage Ⅱ. Conclusion：MSCT has important value in diagnosis and staging of retinoblastoma, which may help to choose which case could be intervention with TOAC.
