高效价冷凝集素患者血型鉴定与输血治疗的研究

目的：探讨高效价冷凝集素患者血型鉴定新方法。方法：对高效价冷凝集素引起正、反定型不合者，采用加热洗涤和冷吸收法，以清除患者红细胞膜上和血清中存在的冷凝集素后，用盐水法作血型鉴定，Polybrine和微柱凝集交叉配血，及时准确地为患者配型输血。结果：2例恶性淋瘤和1例系统性红斑狼疮（SLE）患者血型正、反定型不合，主、次2侧均发生强凝集，不规则抗体呈强阳性；患者红细胞经37℃加热洗涤，于4℃条件下吸收冷凝集素后，血型分别定为Rh阳性AB型2例，0型1例，不规则抗体呈阴性，用Polybrene和微柱凝集配血相合。结论：用加热洗涤和冷吸收可排除高效价冷凝集素的干扰，便于血型鉴定和交叉配血。

Study of blood typing and transfusion in patients with high titer cold agglutinin
