糖尿病大鼠椎间盘中白细胞介素-1α和肿瘤坏死因子-α的含量变化

摘    要:背景糖尿病患者椎间盘突出手术后效果较差,有研究表明与椎间盘组织中有一些炎性细胞因子及其水平的变化有关,但真正的原因和机制尚不十分清楚.目的测定糖尿病大鼠椎间盘组织中白细胞介素-1α(IL-1α)和肿瘤坏死因子-α(TNF-α)的含量变化,探讨其术后效果不佳的可能原因.设计随机对照的实验研究.地点和对象实验在上海九院实验动物中心和上海第二医科大学免疫研究所完成.对象为清洁级雄性SD大鼠36只,购自中科院上海实验动物中心,4月龄,平均体质量244 g.干预措施将SD大鼠随机分为2组,每组18只.糖尿病组用腹腔单次注射链脲佐菌素溶液(streptozotocin,STZ),40 mg/kg,造成糖尿病模型.正常对照组仅给予等量的柠檬酸缓冲液.72 h后测定大鼠的血糖.血糖在16.0 mmol/L以上者确定为糖尿病.分别在1,3,4个月时处死大鼠(每组中取6只),取其腰椎间盘组织采用ELISA法(免疫酶联反应)进行测定.主要观察指标椎间盘组织中IL-1α和TNF-α的含量.结果在干预1,3,4个月时,糖尿病组中IL-1α的含量分别为(1.22±0.28),(1.21±0.21),(1.36±0.28)ng/g,对照组分别为(0.20±0.07),(0.15±0.04),(0.18±0.06)ng/g.糖尿病组TNF-α的含量分别为(0.99±0.20),(1.05±0.19),(1.08±0.19)ng/g,对照组分别为(0.31±0.08),(0.27±0.07),(0.28±0.05)ng/g,糖尿病组IL-1α和TNF-α的含量明显高于正常对照组(P<0.01).两组中IL-1α和TNF-α的含量在3个不同时期差异无显著性意义.结论糖尿病大鼠的椎间盘组织中IL-1α和TNF-α的含量明显增加,并在较长时间内维持在高水平.这些变化可能与造成糖尿病患者椎间盘突出术后效果相对差有关.

Abstract:BACKGROUND: Diabetic patients having received operation for protrusion of intervertebral disc have shown a poor outcome. There are some researches showing that this is related to the inflammatory cells in the tissue of the intervertebral disc, and the changes of their level. But the real causes and mechanism still remain unclear.OBJECTIVE: To measure the content changes of the interleukin- 1 α (IL- 1 α)and tumor necrosis factor-α(TNF-α) in the intervertebral discs of the diabetic rats to explore the possible causes of poor outcome of operations.DESIGN: A randomized controlled experimental study was conducted.SETTING and PARTICIPANTS: Thirty-six male Sprague-Dawley(SD)rats of 4 months(purchased from Shanghai Laboratory Animal Center, Chinese Academy of Sciences) were randomly put into two groups with random number-table method with 18 rats in each group. The study was conducted in the Center for Experimental Animals, the Ninth Hospital of Shanghai and the Institute of Inmunology, Shanghai Second Medical University.INTERVENTION: The diabetes mellitus(DM) was induced by a single intraperitoneal injection of streptozotozin(STZ) solution(40 mg/kg) .In control group, the rats were injected with the sodium citrate buffer of the same volume. The blood glucose level was measured 72 hours later. The standard for the DM is above 16.0 mmol/L. The rats were put to death 1month, 3 months and 4 months later respectively. Each time 6 diabetic rats and 6 control rats were put to death respectively, and the tissue of their intervertebral disc was taken for determination with ELISA.MAIN OUTCOME MEASURES: The contents of the IL-1αα and TNF-α in the intervertebral disc.RESULTS: At the time of one month, three months, and four months respectively after the intervention, the contents of the IL-1 α in diabetic groups were (1.22 ±0. 28), (1. 21 +0. 21), (1.36 +0. 28) ng/g respectively, while in control groups, they were (0.20 +0. 07), (0. 15 +0.04), (0. 18 +0. 06) ng/g respectively. The contents of the TNF-α in diabetic groups were (0.99 +0. 20), (1.05 ±0. 19), (1.08 ±0. 19) ng/g respectively, and in control groups they were (0.31 ±0.08), (0. 27 +0. 07), (0. 28 +0. 05) ng/g. The levels of the IL-1αα and TNF-α in the intervertebral discs of the diabetic rats were significantly higher than those of the control rats( P < 0. 01 ) . No significant differences in the levels of the IL-1 c and TNF-α were found among the three groups of control rats and the three groups of diabetic rats.CONCLUSION: The levels of the IL-1α and TNF-α in the intervertebral discs of the diabetic rats rose significantly and kept a higher level for a long period. These changes could be responsible for the poor outcome of operations on the diabetic patients with protrusion of intervertebral disc.
