乳腺癌相关多原发癌25例临床分析

目的探讨乳腺癌相关多原发癌的诊断、治疗及预后。方法回顾性分析我院自1989年1月至2006年10月经病理组织学证实的25例与乳腺癌相关多原发癌的临床资料,结合文献对乳腺癌相关多原发癌诊断标准、发病率、病因及危险因素、治疗方式及预后进行讨论。结果25例均符合多原发癌的诊断标准,首发癌为乳腺癌的患者术后均行放化疗。异时性癌23例,同时性癌2例。25例多原发癌中,20例首发癌为乳腺癌,次原发癌最多见是肺癌(5例),其次为肝癌(3例),9例仍健存患者中次原发癌为乳腺癌为4例,甲状腺癌为2例,非霍杰金氏淋巴瘤、鼻咽癌及卵巢癌各1例,次原发癌为肺癌和肝癌的生存期均在2年内。结论乳腺癌术后放化疗可能是次原发癌诱因之一,多原发癌的预后与次原发癌的恶性程度相关。

Multiple Primary Carcinoma correlated with Breast Cancer:A Report of 29 Cases

Objective To study diagnosis,treatment and prognosis of multiple primary Breast cancer.Methods The data of 25 patients with multiple primary carcinoma correlated with breast cancer from January 1989 to October 2006 were restrospectively analyzed.To evaluate diagnosis,incidence,etiology,risk factors,treatment and prognosis.Results 25 cases all were confirmed with multiple primary carcinoma,23 cases were metachronous,accounting for 92%,2 cases were simultaneous(8%).Out of 25 patients,20 cases of breast cancer were the first primary carcinoma.lung cancer(5 cases) were frequently occurred in second primary carcinoma,followed by liver cancer(3 cases).9 cases are still in health,of them,4 cases with breast cancer of second primary carcinoma,2 cases of thyroid cancer,1 cases of non-Hondkin lymphoma,nasopharyngeal carcinoma and ovarian cancer respectively.The survival time of lung cancer and liver cancer with second primary carcinoma was less than 2 years.Conclusion radiotherapy and chemotherapy may be the risk factors of multiple primary carcinoma,The prognosis of multiple primary carcinoma was related to the malignant degree of second primary carcinoma.
