| 摘 要： | 目的探讨肺表面活性物质相关蛋白A(SP-A)在急性坏死性胰腺炎(ANP)并发急性肺损伤(ALI)发病中的意义及其可能的机制。方法将健康SD大鼠100只随机分成两组:假手术组(SO组)和模型组(ANP组)。成功制模后,各组分别在6、12、24、36、48 h采用酶联免疫吸附法(ELISA)测定血清及肺泡灌洗液(BALF)中SP-A的浓度。同时进行肺湿/干质量比値(W/D)测定,观察胰腺和肺组织病理形态学的改变。结果 ANP组血清SP-A浓度从6 h开始明显上升,36 h达到高峰,48 h仍强烈表达,数值仍高于SO组(P<0.01)。ANP组BALF中SP-A浓度较SO组相同时间点明显下降(P<0.01)。随着胰腺损伤的逐渐加重,肺损伤程度也逐渐加重。结论血清SP-A浓度的增高及BALF中SP-A浓度减少程度与病情严重程度呈正相关。
